- 1 Proof of Identity (Birth Certificate, GSIS/SSS UMID Card, Driver’s License or Passport)
- Marriage Contract for Married Women
- 1 Proof of Address (Brgy. Clearance, Bank Statement, or Utility Bill)
- Php 504.00 Postal ID Fee
