Bring and photocopy any government issued ID card (SSS, Driver's License, Postal, COMELEC etc.)

To all applicants, visit Cebu City DWUP office, 2nd floor in City Hall to get the application form and waiver.
On September 16, 2018 bring the payment for notary of the waiver. A raffle draw will be held and 10 applicants can win Gift Certificate worth 500 pesos.
